workingtrees on case-insensitive filesystems

WorkingTrees require special intelligence to support case-insensitive filesystems really well. (The main example is Windows, but they are also common on Mac OS X and might happen on Linux.)
We need to allow for files being present in the working tree even when they've changed case from what's stored in the inventory.
