Refactor Storwize Driver for Mitaka

Split init.py file into storwize_common.py, storwize_svc_iscsi.py, and storwize_svc_fc.py. Cleanup unused code and code that is redundant (san config opts and ssh code). Move helpers.py into storwize_common.py. Figure out a good place for the replication code.
