Automatically add breaktables to menuConvert

New break-point tables are currently a bit of a pain to introduce to an IOC. It should be possible to automatically add them to the menuConvert menu when they are loaded, although it can't allow that after iocInit has been run.

This work should also look at: 1) How easy is it to reload tables with different data? 2) Should we add locking to support reloading tables? 3) What happens if LBRK points beyond the end of a new table?
