Support server health monitoring solution based on Monasca

Monasca provides monitoring-as-a-service, this blueprint will support a server health monitoring solution by providing a list of preconfigured actions you can associate with an alarm in Monasca. So it defines action policy and execute the actual action defined in this policy.
Action policy is related to an alarm generated from Monasca, and action will be triggered after an alarm state transformed.
These action policies are operations that occur in response to triggered alarms in Monasca. For example, vm migration is an alarm action when server has issues. They're also considered both physical machine and virtual machine operations.
