evacuation confirmed by migration

In the host failure workflow, it waits for evacuation confirmation by
vm_state and hypervisor hostname changes of the instance. The confirmation
is not simple and brief.
When calling nova evacuate api, it will produce a migration to sync the
evacuation progress. It can confirm the evacuation by the migration' status.
