Add FluentFormatter

I propose an new formatter: FluentFormatter.
It enables oslo_log to output logs into fluentd directly.
FluentFormatter expects it will be used by fluent.handler.FluentHandler.

fluent.handler.FluentHandler is a part of fluent-logger-python.
