clean up the default dictionary's meta commands

The default Plover dictionary contains many syntactically correct meta commands that produce undesirable output. For example, {} is extraneous and {& B} should probably be {&B}. These and many other dictionary entries need to be reviewed and fixed by a stenographer who understands the original intent of the entry.
