Improved handling of APT cdrom: sources

As of Ubuntu 5.10, the default APT sources.list will contain a cdrom: source corresponding to the installation medium. This results in counterintuitive behaviour when the user requests the installation of a package. Often, the user has discarded, overwritten or stowed the installation medium, or for some other reason would prefer that the package be fetched from the network.
