Rename "Title" to "Name" and "Label"

The field "Title" in the contact creation dialog should be renamed as "Name" (so it will be contact "Name" and "Description" and not "Title" and "Description". All occurrences of "Title" fields in Addresses, Email Addresses, Fax Numbers, Notes, Phone Numbers, and Websites should be changed to "Label".
