Convert all actions to new-style actions

The various CMF packages currently define actions in three ways:

 - some are defined as so-called "new-style action" objects inside the portal_actions tool

 - some are defined on tools and type information objects using the older-style action machinery

 - DCWorkflow stores action information on the Worklist and Transition classes in an entirely different way that uses string replacement instead of TAL expressions to define action and icon URLs

All actions should be done as "new-style" actions.
